You hereby make this application for membership in the Kingsley Racquet & Swim Club, Inc. If accepted for membership, You agree to abide by the present Charter and By-Laws of the Corporation as adopted by the incorporators, and amendments to said documents, as may be promulgated from time to time, and to cooperate with the rules and regulations of the Club as proscribed by the Board of Directors.

You hereby understand that should you terminate Membership with the Club for any reason, no part of the initiation fee or dues shall be refunded.
